Position Title:
CLINICAL PHARMACIST (Inpatient) Work Schedule:
Monday through Friday, 8:
00am to 4:
30pm (Primary) ; Alternate shifts, weekends, and Holidays may be required based on operational needs of the hospital. Incumbent serves as a Staff Pharmacist and is responsible for the total range of pharmacy work which includes interacting clinically with health professionals, consulting and educating both patients and health professionals, and processing/dispensing medication orders. Incumbent has extensive knowledge and experience in pharmacy and a broad knowledge of the various aspects of a total pharmacy operation including experience with unit dose, IV admixture, and interdisciplinary rounds with physicians. Incumbent will also participate on various committees, review charts, monitor drug therapy, provide pharmacokinetic adjustment of medications, performs clinical assessments, and provides drug information, discharge counseling and other pharmacy duties as assigned. Performs patient medication profile reviews upon entering orders and utilizes knowledge of pharmacology, pharmacokinetics, pharmacodynamics and pharmacotherapeutics to evaluate orders for appropriateness. Ensures appropriate laboratory monitoring of drug therapy and provides pharmacokinetic services. Follows guidelines, protocols, and policies in performing these services. Provides medication and disease state management information and participates in order educational endeavors including patient and family education and medical/nursing staff education. Enters and/or edits new prescription orders from providers accurately. Process refills and renewal prescription requests in a timely manner. Accurately dispenses medication orders and supplies. Checks technician-prepared orders when necessary. BASIC REQUIREMENTS:
1. Citizenship:
Must be a citizen of the United States. 2. Education:
Must be a graduate of a degree program in Pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE). 3. Licensure:
Must have a full, current and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States or District of Columbia. 4. Physical Requirements:

Must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), 7407(d). NOTE:
The full performance level of this vacancy is GS-12. The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate. GRADE DETERMINATIONS:
At the GS-12 level, in addition to the basic requirements, candidates must meet one of the following:
Experience or Education. 1. One (1) year of experience equivalent to the next lower grade level GS-11, or 2. Completion of an ACPE accredited Pharm.D. program. Experience is only creditable if it is post-licensure experience as a professional pharmacist directly related to the position to be filled.
Experience:
To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. MINIMUM QUALIFICATION REQUIREMENTS:
In addition to meeting the basic requirements, pharmacists at the GS-12 level must demonstrate all the following Knowledge, Skills and Abilities(KSAs):
1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred:
1. Skill in writing progress notes, completing medications reviews, and working with interdisciplinary teams, especially for adult population. 2. Knowledge of pharmacokinetics and adjustment of vancomycin, aminoglycosides, warfarin/anticoagulants, and other antibiotics. Applicants must meet the physical standards for the position. The work requires sitting, walking, standing, bending and stooping, pulling an pushing and lifting/carrying of items up to 20 to 50 lbs. and be able to maintain selfcontrol in difficult stressful situations. A physical examination prior to placement is required. The full performance level of this vacancy is 12.
