The RN position located in Waco, Texas It is a fast paced daily triage position to receive calls from patients/families, where the RN is responsible for planning, organizing, coordinating and providing professional nursing care for patients in a triage ambulatory care setting. This RN uses advanced clinical, interpersonal, and communication skills to assist patients/families to use services appropriately, to enhance the continuity of care and to achieve desired patient outcomes. Major responsibilities include:
Provide professional nursing assessment and intervention to patients with acute and chronic care needs. Provide professional nursing care for patients in a triage ambulatory care setting following established standards and protocols, while incorporating a high level of critical thinking skills. Supports quality patient and family-centered care principles through performing a variety of high level functions within a care team to support the needs of the healthcare team and patients/families. Coordinate care among services for veterans with complex health care needs assigned to Primary Care or requesting clinical services via telephone or video communication. Educate patients and family about various medical conditions, maintain complete, timely and accurate clinical documentation for each patient and continually look for opportunities to improve processes and workflow. Support patients/families by providing multiple options for seeking healthcare and providing evidence based best practice education regarding medical conditions. Serve as a resource and advocate for patients/families healthcare needs. Collaborate with members of interdisciplinary teams to influence care outcomes. Provide competent, high quality, safe, efficient and effective telephone care in stressful situations Ability to work cooperatively as a member of a team in all assignments and self-motivated in the pursuit of meeting professional work standards while providing high quality patient care. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Grade Determinations:
